Retrocalcaneal bursitis or insertional Achilles tendinitis
Retrocalcaneal bursitis and insertional Achilles tendinitis are less commonly mistaken for a calcaneal stress fracture, but it still happens when clinicians did not perform a thorough check. The distinction is straightforward: pain at the back of the heel rather than underneath the heel, with tenderness directly at the Achilles tendon attachment.
